TCP People - Transforming Care newsletter

Dear Accord member,

Welcome to the first edition of TCP People, the Humber Transforming Care Partnership (TCP) newsletter. This quarterly newsletter will update you on how Humber TCP is working to change health and care services in a way that will make a real difference to the lives of children, young people and adults with a learning disability and/or autism in Hull, East Riding of Yorkshire and North East Lincolnshire.

The Humber TCP involves:

  • People within the learning disability community in East Riding of Yorkshire, Hull and North East Lincolnshire
  • NHS East Riding of Yorkshire Clinical Commissioning Group (CCG)

  • NHS North East Lincolnshire CCG
  • East Riding of Yorkshire Council

  • NHS England Yorkshire and Humber Specialised Commissioning Team
  • National Probation Service - Hull & East Riding

In order to ensure we reflect all the work happening across our Humber TCP area we welcome any suggestions you have for articles, news and developments you would like to see covered in future editions.
